Health insurance becomes a practical question as soon as you sign a German employment contract.

Your status decides the system

Private Health Insurance in Germany: Cost and Monthly Prices

Private health insurance does not charge a percentage of your salary. Your premium depends on your entry age, your health and the cover you choose. The premiums are health insurance only, without long term care insurance. Self employed people and freelancers pay the first column, employees the second.

Employer share

For employees the employer pays about half of the eligible premium, up to the statutory maximum. Above that ceiling your own share grows.

Compared with GKV

Statutory insurance charges a percentage of income up to the contribution ceiling. Private insurance charges an individual premium instead.

Contributions are not fixed for life and can increase later.

Contribution refund

A Beitragsrückerstattung depends on the conditions of the individual tariff and usually on claiming no benefits.

Private Health Insurance in Germany: Salary Limit and Eligibility

Whether you may choose private insurance depends on your employment status.

In 2026 the compulsory insurance limit (Versicherungspflichtgrenze) is €77,400 per year. Above it you may choose between statutory and private insurance, below it statutory insurance stays compulsory. Passing the limit during the year ends the obligation with that calendar year.

The salary limit does not apply. Private insurance is open regardless of income, unless a separate statutory obligation applies.

Beihilfe covers part of the medical costs. Private insurance covers the remaining share.

How We Choose the Best Private Health Insurance in Germany

We compare six insurers for expats: ARAG, LKH, Allianz, ottonova, Hallesche and HanseMerkur. Three criteria decide which of them fits you.

Premium stability

The entry premium says little about what you pay in ten years. We look at how an insurer has adjusted premiums in the past and how the tariff is calculated, because that development carries the real cost.

Support in English

Signing a policy in English is common. Claims, invoices and medical questions are where the language matters, and not every insurer answers in English there. We check the service in the claim, not only in the sale.

Waiting periods

A waiting period is the time between the start of cover and the first claim for certain benefits, often dental treatment or psychotherapy. Some tariffs drop it after a medical examination. We check where a period applies and what removes it.

Portal or broker

A comparison portal sorts by price, and that answers one question well. We sort by fit: your entry age, your health record, the benefits you need and how the insurer behaves years later. That order often puts a different tariff first.
